Staging Laparoscopy

What is Staging Laparoscopy?

A laparoscopy is a specialist procedure performed under general anaesthetic. It allows the surgeon to insert a small keyhole camera in to the tummy cavity and to have a look around without the need for large incisions. It provides a more direct way of assessing the contents of the abdomen and pelvis compared to CT, PET or MRI scans.

Why do we use laparoscopy in patients with peritoneal malignancy?

Cytoreductive surgery (CRS) and Hyperthermic Intraperitoneal Chemotherapy (HIPEC) achieves the best results for patients if the surgeons are able to remove all the abnormal tissue during CRS and HIPEC. In most cases what stops the surgeons removing all the abnormal tissue is if the small bowel is heavily involved. Unfortunately scans don’t always detect the extent of small bowel involvement and therefore a direct visualisation of the abdominal and pelvic cavities is indicated to establish if the patient will benefit from CRS and HIPEC. A laparoscopy is organised to assess and document the extent of peritoneal disease and this helps the surgical team to establish if CRS and HIPEC will be beneficial and also gives the chance to tell the patient and their family the extent of the proposed operation.

How is Laparoscopy performed?

The patient is put to sleep (general anaesthetic) by the anaesthetists so no pain is felt during the procedure. The surgeon then makes a small 10mm incision through which a camera (laparoscope) is inserted in the abdomen. The abdomen is then filled with CO2 gas to allow space for the surgeons to get good views of the abdominal and pelvic cavities and the organs within. One or two further 5mm incisions are made to insert other instruments. The surgeon then assesses the abdomen and pelvis. If required biopsies can be taken from abnormal tissue to help plan treatment. At the end the CO2 gas is released and the incisions are closed with sutures. Patients usually go home the next day after discussing the results of the laparoscopy with their surgeon.
